Hospital Income

To ensure prompt processing of your claim, it is important that you submit a completed Hospital Income Insurance Claim Form with the relevant documents to:

Enclosed documents:

• Prescriptions
• Medical documentation
• Hospital Admission 
• Hospital discharge
• Surgery report/certificate
• Incident report
• Registration card and Driving license in case of traffic accident
• Police report
• Relevant others

The above mentioned relevant documents must be submitted to the Company within 30 days from the date of occurrence of the events resulting in the Insurance Claim.
